Apply for your first passport or renew a youth passport
The menu below provides more information about what you will need to bring to complete your passport applications for specific situations.
We offer passport photos for $20 and try to provide timely and same day appointments to eligible applicants.

Please note that while we have all paper passport forms available, passport service is by appointment and the upstairs office is not always staffed. Some specific forms can be requested as not all are out in the lobby of the courthouse, if you let us know, we can leave a copy of the form you need with the guards. Many forms are also available at the Burlington Post Office as well.

Will need:
A completed DS-11 form
A certified Copy of Birth Certificate and a photocopy of the birth certificate (or an expired, previously issued passport). If you were born in Vermont, most city clerks can print a certified copy.
Both parents present with valid Vermont (for federal identification) (or out of state ID and secondary ID) and photocopies of front & back of all IDs presented. (If solo parenting, some proof of this like a birth certificate listing only one parent or an official court order granting one parent sole custody).

A check or money order payable to Department of State for passport fees.
Check or cash for execution fee payable to Chittenden County.

PLEASE NOTE applicants age 16-17 years old should apply with one parent present with ID.

Check to see if you are eligible to use the State Department’s Online Renewal Tool
OR
Complete form DS-82 and include in your envelope:
Your expired or expiring passport
Passport photo taken in the last 6 months
Check for $130 payable to the Department of State, $190 if expediting, $212.05 if expediting with 1-2 day service.
Mail the form with these items to the address on the DS-82 form. Please mail in a flat envelope and use a traceable delivery method.

DO NOTE: If your passport is more than 5 years expired, you will be prompted to apply in person with the DS-11 form at a passport acceptance facility like ours.
